1. In the Forms module, in the Actions column, click the ellipsis  for the form you wish to copy and select Copy Form.

   

2. A window will appear asking "Are you sure you want to Copy"; click OK to confirm you wish to make a copy.

   

The form will be displayed in your list of forms, name Copy of...
