For each Fundraiser, you can add/edit the fields you wish to include on your application form. Custom Fields must be created before selection in the Fundraising module. 

1. In the Fundraiser, select the Setup tab. 
2. In the Custom Field section, select the plus icon.

3. Select the custom field you've created from the dropdown menu and select Done.
